This evaluation is to help the Salem Basketball gather feedback on the coaches for the current year.  Salem Basketball wants to make sure that both parents and players have had a good experience, by filling out an evaluation for your coach you will be helping the Salem Basketball verify that they have made the correct choices for coaches.

This form is used to get information on any person who would like to coach for Salem Basketball at any grade and in any program.   Coaches coaching in either the recreational league or travel league must complete this form each year.

Any candidate selected to Coach a Salem Youth Basketball Team will be subject to Background Check and must attend all Coaches Clinics.
